Minimum Requirements
You must be 18 years of age or older.

You must be physically able to perform and demonstrate manual massage techniques.

You must be currently certified through Madriella.

You must have at least 5 years of verifiable experience as a Doula. We may request copies of your initial certification/diplomas for this purpose. (The 5 years DO NOT have to be with Madriella).

You must be willing to submit the names and contact numbers of some of the women you have attended as a doula during birth, the more the better.

We want only experienced and knowledgeable women teaching Madriella students. In the space provided below please describe your experience as a doula, including how many births you have attended, what formal education you have, and what additional certifications you have (other than your Madriella Certification). If you have teaching experience please describe that as well.
References
In the space below please give us the name and telephone number of some of the mothers you have attended as a doula during the birth of their children. Please list as many as possible, but make sure you have their permission first, we will be calling them.
If you are approved as a Workshop Provider you will be required to email us the following:

1. A profile picture (for your Madriella.com Applicant Registration Page).

2. The City and State where you intend to hold your Workshops (for our directory).

3. Contact Information (A business phone number, facebook page, etc for your Madriella.com Applicant Registration Page).

4. A link to your website (if you have one).

Use of the Name Madriella and the Logo
If you are approved as a Madriella Doula Workshop Provider you will be granted permission to use the title "Approved Madriella Workshop Provider" In your advertisements. Use of the Official Madriella logo for workshop advertisements will be covered in your new Approved Provider information packet. The title and logo are to be used only in the manner specified in the Approved Provider Packet.

You are not authorized to use the name "Madriella", "The Somatic Arts and Sciences Institute" or "Approved Madriella Workshop Provider" in any contract or form, including those used in renting of a facility to host a workshop.

In other words, you are not authorized to make agreements as agent or employee of the Somatic Arts and Sciences Institute, Madriella.com, or their owners

Liability Waiver
The Approved Workshop Provider is legally responsible for any and all expenses associated with the workshop, and that includes buying enough birth balls for everyone to use (a minimum of 1 ball for every two guests- a one time expenditure), printing out any flyers or advertisements, providing refreshments, renting a facility (if not in your home) etc.

The safety of the participants is the responsibility of the Approved Workshop Provider.

The Somatic Arts and Sciences Institute, Madriella.com, and their owners are not responsible for any financial obligations incurred by a workshop host and is not responsible for any damage to persons or their property attending the workshops. All financial responsibility lies with the Approved Workshop Provider Host.

Nonemployment Statement
The Approved Workshop Provider is an independent agent being granted permission to use the Madriella name and curriculum to host workshops and is not an employee or contractor of the Somatic Arts and Sciences Institute, Madriella.com, or their owners.

Granting of the title of Approved Workshop Provider does not entitle the Approved Provider any of the benefits commonly associated with employment or contractual work, including but not limited to: Workman's Compensation coverage, Wages, or issuance of a W2, W4 or any other Tax documentation.

Payment for Workshop Hosting
The Approved Workshop Provider is paid by the individuals attending the workshop, not by the Somatic Arts and Sciences Institute, Madriella.com, or their owners. You are responsible for collecting your own payment from the attendees and for claiming said revenue on your income tax. The Somatic Arts and Sciences Institute, Madriella.com, and their owners cannot and will not assist you in claiming money owed to you by attendees.

Workshop Content
To protect the safety of the attendees and the integrity of the program the Approved Workshop Provider is only to teach the approved curriculum in the Workshop. If the Workshop Provider would like to teach additional material not included in the approved curriculum they may do so only after they have concluded the official Madriella Doula Workshop and have made it clear to the attendees that the additional material is not part of the Madriella curriculum.

For example:
If you wished to include a block of instruction in making plaster casts of a mother's tummy you are free to do so, but you must formally end the Madriella Workshop and give the attendees an opportunity to leave.

Naturally you would need to differentiate that it is a separate Workshop in any advertising as well.

Exception
The teaching of any clinical or medical tasks is never to be offered alongside, before or after any Madriella Doula Workshop. This includes Leopold's maneuvers, Checking Blood Pressure, Checking for Dilation, etc. Doulas are non-medical personel and offering any workshop content that is considered clinical or medical is grounds for immediate revocation of Approved Workshop Provider Status and may result in legal ramifications for the Workshop Provider.

Attendee Registration and Certificates of Completion
All people attending an Approved Providers Workshop must register with the Approved Provider through the Approved Providers webpage or another means arranged by the Provider.

In order to receive their certificate of Completion for the Madriella Doula Course they must take and pass the online certification examination on the Madriella.org website.

All official Madriella Doula Certifications are numbered and must be issued by the Madriella home office.
